No long-term contracts, no commitments, and you may cancel your membership at any time.
Members pay membership fees as a recurring monthly payment via drafts against a credit card, debit card, bank account, or suitable alternative. Membership begins once enrollment is complete and the first month’s payment, enrollment fee is received, and after a 5 day processing period.
Members pay for a six month membership in advance and receive a 5% discount.
Members pay for full year membership in advance and receive a 10% discount.
This plan is for individuals, couples, or families with either a high-deductible insurance plan or no insurance, or for those who simply desire ready access to quality care. It provides each member unlimited access (Healthcare Direct) to these services provided at Prestige Urgent Care:
Members can walk in or make a same day appointment to receive urgent care services. They also can call or email our clinic concerning healthcare issues that require attention or to ask questions about a healthcare issue or treatment. Sometimes a simple call can save a clinic visit.
This plan provides unlimited access to urgent and primary care for children under the age of 18.
Prestige Pediatric is tailored for families where the parent(s) already have healthcare and wish to cover only their children, or for cases where their insurance only provides Medi-Cal coverage for their children.
A one-time enrollment fee of $59 will be charged for enrollment.
Monthly memberships and prepaid memberships become active after a 5 day processing period.
Monthly members can cancel their Prestige membership at any time. However, you must submit a written Cancellation Notice. Complete the notice, fax or mail it to us, or drop it off at our clinic. Your membership will be cancelled on the last day of your monthly payment cycle, but we must receive your written cancellation notice at least five days before the end of your due date for the next payment cycle. Otherwise, you will be billed for the next month of membership.
Prepaid memberships are non-refundable. They cancel automatically at the end each prepaid period. Members are notified 30 and 15 days before their membership expires. They may renew their membership on a prepaid basis or convert to a monthly membership.
Prestige membership is not medical insurance. Our members receive unlimited access to high quality primary and urgent healthcare. However, Prestige membership only covers the services offered on site at our clinic. Therefore, we recommend that our members also carry some form of health insurance policy to cover events such as major surgery, hospitalization, or a serious illness. For many, the combination of a high-deductible healthcare insurance plan and a Prestige membership represents the most cost-effective solution.
